Expanded Prenatal Care

WIN’s Obstetrics Program uses proprietary protocols to achieve significant reductions in pre-conception and prenatal care costs due to optimal clinical utilization, elimination of unnecessary services, reduced incidence of inpatient utilization for pre-term labor, increased support services including provider and member education and improved access to care providers.  

WIN’s Care Management program is staffed by experienced OB/GYN clinicians who provide intensive, hands-on case management. Our Care Managers are Obstetric nurses with years of experience in clinical practice. They exclusively manage a small number of OB cases and serve as the central control point within our program. Responsibilities include initial triage of each Obstetric patient; coordination of care with provider based on protocols; continuous review for patient progress and treatment plan revisions; patient education and moderation of expectations; and analysis of key quality indicators of obstetric programs.

Targeted Care Management

WIN’s Care Management approach is an evidence based process of organizing and coordinating resources and services in response to individual healthcare needs along the continuum of care. Requests for tests, diagnostics, treatments, procedures and pharmacological interventions are evaluated based on medical necessity and proven efficacy in the scientific community. The WIN Care Management team plays a critical role in developing, monitoring, and evaluating the plan of care and utilization of resources to reach mutually agreed upon clinical goals and outcomes in a specific time frame. The key to the success of the Care Management program lies in the experience of the registered professional nurse and the Medical Director working in a collegial relationship with the community physician on a case specific encounter.
